Contact form
A contact form beside the other ways to reach you – email, phone, chat, address, opening hours and Instagram. Messages go to your store's email address.

Set it up
- Write the Heading and Text – when you reply and what to include.
- Choose which fields to show: Show phone field and Show order number field. Name, email and message are always there.
- Add Contact channel blocks for your other channels: pick an Icon (email, phone, chat, location, opening hours, Instagram), a Title, a Text and a Link.
Where messages go
Messages are sent to the sender email in Settings › General. Shopify protects the form against spam, and may show a captcha to some visitors.
Layout
Layout: Text left, form right, or Stacked. The Contact page template places a FAQ under the form, so common questions get answered before they are asked.
